Abstract: Form CCC-434 TAP Application for Assistance is used to record requests for losses or damage to eligible trees or vines. Includes the eligibility certification statements: 1) Had an annual qualifying gross revenue of less than 2.5 million for the preceding taxyear; 2) allows FSA representatives access to fields; 3) complete replacement within 12 months from date of application; 4) refund any overpayment. CCC-435 TAP Technical Worksheet is used for the collection of data and calculation of losses of trees or vines due to natural disaster.
