Approved for use
with the clarification that all requirements in this submission
(with the exception of 42 CFR 489.20 (m) which relates to
investigatory activities) continue to be covered by the PRA, but
impose nominal burden as "usual and customary" pursuant to 5 CFR
1320.3 (b) (2). OMB requests that HCFA continue to evaluate in the
future whether these requirements remain "usual and customary" for
the hospital industry.
