In accordance
with 5 CFR 1320, the information collection is approved with the
following terms of clearance. Questions on every form relating to
race and ethnicity shall be revised to reflect current guidance on
Standards for the Classification of Federal Data on Race and
Ethnicity (published in the Federal Register October 30, 1997,
p.58782-58790. The agency will submit the revised forms with a
change worksheet. Continued use of forms with existing
race/ethnicity questions shall constitute a violation of the
PRA.
