To verify that powered industrial
truck operators are evaluated and trained to perform their duties
competently and safely. To comply with the evaluation and training
requirements, employers must prepare records certifying that their
employees have successfully completed powered indusrial truck
operator training.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
