This collection
is approved until the end of 1999. The agency must explain why the
number of respondents increased from 4,289 to 7,785. This change is
not described in the supporting statement. Nor is an explaination
for the drop of reported burden per response from 1 hour/response
to 1/2 hour. Until this change is explained, this approval assumes
the previous 1 hour/response. In addition the form must be modified
to include a Paperwork Reduction Act statement which informs the
public of the significance of the OMB control number and the other
required information described in 5 CFR 1320.8(b)(3).
