Terms of Clearance: The agency has stated that no statistical methods will be employed in the conduct or analysis of this survey. Thus, the agency may not report numerical results for this survey or present such results as representative either of the visitors to a particular forest, users of a particular service, or the population as a whole. If the agency should desire to present statistical averages of its results or present their results as representative of a larger population, the agency will need to take steps to properly sample the larger population (including restricting access to the survey) and ensure a sufficiently high response rate from those sampled. Such modifications to the survey would require submission to OMB of a revision to this collection. Further, if the results of this survey are intended to provide GPRA performance measures, the collection should be resubmitted to OMB with revisions to make the results representative of its customers.

Abstract: These comment cards monitor satisfaction with existing services of the Forest Service in accordance with E.O. 12862. Customers are asked if they received prompt, courteous services; if they were provided with the information or service they needed; if they felt that, for their request or business, the procedure was clear and efficient; and if they were satisfied with the facilities used. They are given opportunity both to rate the service and to offer suggestions/comments. Respondents are individuals who are the direct beneficiaries of Forest Service services.
