The FCC has
permission to use an edition date in lieu of an expiration date for
this collection. As requested, the FCC is granted generic approval
for this collection -- but are required to submit a copy of the
revised collection instrument along with an OMB Correction
Worksheet when changing these forms. The FCC shall, however, submit
all significant revisions of this collection to OMB for review and
approval under 5 CFR 1320.

The USOA Report provides the annual
results of the carriers' activities for each account of the Uniform
System of Accounts. The Service Quality Report provides service
quality information in the areas of interexchange access service,
installation and repair intervals, local service installation and
repair inter- vals, trunk blockage, and total switch downtime for
price cap companies. The Infrastructure Report provides switch
deployment and capabilities data.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
