This collection
is approved as amended by NCHS's revisions of 6/10/99 and per the
agreement reached with NCHS on 6/22/99 (also documented in NCHS's
message of 8/27/99). This collection is approved through 3/31/2001,
covering survey years 1999 and 2000. NCHS will send a new package
to OMB in year 2000 covering survey years 2001 and 2002. Consistent
with the agreement reached, NCHS agrees to do the following: (1)
notify OMB of any minor changes to a package (e.g. wording changes
to questions) while the survey is in the field. Notification shall
be done through a letter. OMB will have 10 working days to respond
to the changes. (2) New components or substantial revisions shall
be submitted with the full package under this clearance number. (3)
Notify OMB of revisions to testing and diagnostics informally
through a letter which OMB will have 10 working days to review.
Changes falling under the informal reivew should be made within
reason. (4) This approval also serves as a generic approval for
pilot testing new components. (As mentioned earlier, new components
will be reviewed formally by OMB once they are incorporated into
the survey.) NCHS shall notify OMB of the information to be pilot
tested; OMB will have 10 working days to review the submission. (5)
NCHS will send revised consent forms to OMB following IRB
approval.
