This collection
is approved, consistent with the changes in the CDC memo dated
10/7/99, and subject to the following conditions: CDC will attempt
to improve the effective response rate. As a part of these efforts,
CDC will contact the National Center for Education Statistics, to
ensure that they are maximizing response rates at every stage of
sampling,(especially at the state and district levels)._If CDC does
not achieve an effective response rate of atleast 70 percent,_or
higher, then any publication of results must include a disclaimer
that cites the achieved response rate, and a detailed explanation
of the factors that prevented the study from achieving a higher
effective rate of response.

Personnel from all 50 States, the
District of Columbia, and nationally representative samples of
district and schools will be surveyed about school health policies
and programs.
