In addition, the following terms apply: (1) In the event that any changes are made to this form as a result of regulations implementing the 1998 HEA Amendments, ED will submit the changes for OMB review prior to the 7/1/2000 effective date for the HEA regulations. (2) In the next submission of this form for approval ED will: (a) have the form use a single numeric identifier for postsecondary institutions, (b) allow for an electronic signature alternative, (c) revise the application process so that all materials can be submitted through an electronic medium.

Abstract: The Higher Education Act of 1965, as amended, requires postsecondary institutions to complete and submit this application as a condition of eligibility for any of the title IV student financial assistance programs and for the other postsecondary programs authorized by the HEA.
