The FNS-191 form is used by CSFP local agencies to collect racial/ethnic data on each participant for 1 month of the year. The FNS-101 form is completed by FDPIR and FSP local agencies on all households for 1 month of the year. The data is used to monitor local agencies' compliance with the Civil Rights Act.
