Abstract: FCC form 464-A is a cover sheet to be used to transmit Phase II unserved area applications by those seeking authority to operate a cellular radio station. The applicant must certify on the form that the application is complete and contains all information required by the Commission's rules. FCC form 464-A will assist Commission staff in processing cellular applications expeditiously.
