The agency must
make the following changes to these forms: -- include clear
instructions as to where to submit completed forms so that the
public stops sending applications to OIRA for processing; --
conform all race/ethnicity questions to OMB statistical standards,
including the option of selecting 'one or more' races; -- clarify
that SBA should not use previous versions of these forms until
exhausted because previous forms do not have a currently valid OMB
control number; -- investigate whether Privacy Act protections
apply to the management of the applicant business and include the
appropriate Privacy Act statements and procedures for this
information. The agency should also explain precisely what they do
with the information on the management of the company, including
the race, ethnicity, sex, and military service information.
