This collection
is approved for two years. OMB notes however that the agency's
response for question 8b in the supporting statement, that
consultation is unnecessary, is not acceptable. The designs of both
forms, as well as the instructions (or lack of instructions as is
the case for Form 90-1), leave significant scope for improvement.
The agency is instructed to reconsider its belief that consultation
is unnecessary and should endeavor to make improvements to these
forms prior to the next submission to OMB. The agency is also
reminded of the requirement to inform the public of the
significance of the OMB control number.
