The Office of
Human Development Services has been collecting the information
contained in this request since 1998 in violation of the Paperwork
Reduction Act of 1995. The agency will take steps to ensure all
requests for approval are submitted to OMB in a timely fashion. In
addition, the agency will include a Paperwork Reduction Act
statement with burden estimates and an OMB control number on all
application materials sent to States.

Under section 1130 of the Social
Security Act, the Department of Health and Human Services is given
authority to approve up to ten States for Child Welfare
demonstrations in each of the five fiscal years 1998-2002. Child
Welfare demonstration projects operating under this waiver
authority are required to be consistent with the purposes of
existing law, cost-neutral to the Federal government, and
independently evaluated. This authority provides an opportunity for
States to design and test a wide range of approaches to improve and
reform child welfare. Information collected from these
