This paperwork
lets contractors report safety issues through a chain of command
that is seperate from their usual supervisory path. This paperwork
is granted an extension for 3 years and the reporting burden is
revised to reflect the correct number of hours, 19, that should
have been entered in the clearance from 1997.

voluntarily and confidentially report to an independent agent, any
safety concerns or hazards pertaining to any NASA program or
project, which have not been resolved through the normal
process.
