In accordance
with 5 CFR 1320, the information collection is approved for a
period of two years (based on the length of the program described
in the supporting statement). However, the agency is directed to
revise the instructions and forms to instruct respondents that they
should not complete or mail the form if the eligibility questions
indicate ineligibility. The agency is also reminded that care
should be taken in calculating burden hours. The requested burden
hours do not accurately reflect the total burden imposed by the
collection. The numbers have been adjusted to reflect the burden
associated with the 5 minutes of burden identified on the
form.
