Terms of Clearance: This information collection request is not approved. The ICR package provided does not contain sufficient information for review. In the supporting statement, reference is made to_"the survey form," however, no form was included in this package. DOE fails to discuss the information that will be collected and the methodology used to collect that information. In addition, the ICR does not include and/or properly address the following: (1) An adequate description of how the information will be used. (2) Sufficient information about duplicative information. (3) Demonstrated compliance with the Privacy Act (4) An estimate of the hour burden indicating the number of respondents, frequency of response, annual hour burden, and an explanation of how the burden was estimated. (5) An estimate for the total annual cost burden to respondents or recordkeepers resulting from the collection of information (see supporting statement instructions). (6) Estimates of annualized costs to the Federal government, including a description of the method used ot estimate cost. (7) A justification not to use statistical methods as a mechanism to reduce burden. Finally, DOE failed to follow the instructions for completing the OMB 83-I.
