The Workplace Helpline is a toll-free,
telephone consulting service of SAMHSA's Division of Workplace
Programs which provides information, guidance and assistance to
employers, community-based prevention organizations and labor
offices on how to deal with alcohol and drug abuse problems in the
workplace. In was required by Presidential Executive Order 12654,
and has been operating since 1987. The Call Record Form is used to
document key characteristics of callers. The Technical Assistance
Follow-up Survey Form is used to obtain feedback on experience with
the Helpline and its services after..
