Terms of Clearance: This collection is approved as a reinstatement with change. The expiration of this collection was due to an error in OMB process- ing and will not be counted as a violation under the PRA. This new format for the VHA customer satisfaction surveys is approved for 13 months. VA will track all surveys developed using this pool of questions during that time and will include them with the resubmission of this package. Upon resubmission, OMB will evaluate whether this new format is working within the intent of the PRA._Furthermore, any additional questions used in the customer satisfaction surveys will need to be approved by OMB
