212(a)(1)(A)(i) or 212 (a)(1)(A) (iii)(1) or (11)of Immigration and
Nationality Act
Form used to meet condition of waiver
filed by alien found inadmissible under Section 212 of the
Immigration and Nationality Act for mental/physical disorders with
associated harmful behavior. Health care providers in the U.S. are
chosen by alien or alien's family, and by completing form, agree to
evaluate alien if waiver of medical condition is granted by the
Immigration and Nationalization Service. Also, agree to provide a
written report of evaluation rto CDC if waiver is granted. Form
must also be completed by alien or alien's family member.
