This package is
approved for 3 years; the disability discharge form will be used
until July 1, at which point a revised disability discharge form
(to be cleared at a later date, but prior to July 1, 2002) will
take the place of the form cleared under the current OMB number.
The other discharge forms will continue to be cleared under this
OMB number for the duration of the 3 year clearance.

These forms will serve as the means of
collecting the information necessary to determine whether a FFEL or
Direct Loan borrower qualifies for a loan discharge based on total
and permanent disability, school closure, false certification of
student eligibility, or unauthorized signature. The school closure
discharge application may also be used by Perkins Loan borrowers
applying for a closed school discharge.
On behalf of this Federal agency, I certify that
