Approved for 6 months under emergency procedures. DOL has indicated their plans to consolidate these forms with those cleared under 1205-0342 and 1205-0339. DOL shall submit the consolidated forms with enough lead time to allow OMB review prior to expiration. DOL will work with OMB in the interim to make changes to the customer survey procedures to increase the methodological rigor of those procedures, and shall explain the standardized methodology for customer sample selection, determinations of when a customer survey is required, and analysis plans for customer survey data. It is noted that, in this package, DOL is continuing the use of ETA 8562 and adding form ETA 8562a.

Information required for Secretary of Labor to make determinations of eligibility for petitioning workers to apply for transitional adjustment assistance under the Trade Act of 1974. The Trade Act of 2002, H.R. 3009, P.L. 107-210 repeals the NAFTA-TAA, but extends TAA eligibility to secondarily affected worker and to workers whose firm has shifted production to certain foreign countries.
