Terms of Clearance: OMB requests that the agency prepare a separate package to include all evaluation forms. This package should include a statistical methods write-up for any survey including questions with quantifiable answers, including 5-point or 7-point scale questions. The agency should also provide historical information about the response rate of participants to these questionnaires. In addition, the race/ethnicity questions on all evaluation forms must be changed to comply with the Race/ Ethnicity standards issued by OMB in 1997. This includes changing the questions into the two question format and allowing the respondent to select one or more. The questions should also be rephrased to clarify for the respondent the appropriate answer where a company is owned by more than one person.
