Terms of Clearance: This information collection request is approved under the following conditions: 1) The advanced letter and survey questionnaire will be revised consistent with e-mails sent by the CNCS dated 7/15 and 7/16. 2) CNCS will not begin pre-calls until the survey instrument pretest has been completed and final revisions to the instrument submitted to OMB for approval using the change worksheet (OMB- 83C). 3) A one-time incentive of $50 is approved for refusal conversion purposes only. When the study is completed and data are available, CNCS should report to OMB (informally via email) the initial participation rate prior to an incentive offer, the response rate by incentive and non-incentive, and the final effective response rate. Summary data on the disposition codes should also be reported to OMB.
