Abstract: Military members and their families are offered assistance in adapting to military life through the programs and services of the Family Support Center (FSC). These forms are used to collect demographic and personnel data, and record services provided to those who use FSC programs and services. These forms provide usage and trends for program evaluation, targeting, and budgeting as well as reports to DOD and Congress.
