Abstract: This form is used to collect information from individuals applying for an Air Force Reserve Officer Training Corps (ROTC) Scholarship Actions Program Scholarship. It is used by Air Force FOTC to process and manage selected students for acceeptance into the program and to select individuals to receive college scholarships.
