Approval is
given for the revised incentive payment as specified in the April
29, 2004 email from NCHS (to pay $5 prepay and $30 upon survey
completion). NCHS should provide feedback to OMB on the
effectiveness of the incentive approach including response rate
achieved. Approval is only given for the resident survey; NCHS will
request via a letter to OMB if the discharge survey is to be
fielded. Also, changes to the instrument should be made according
to the April 28, 2004 NCHS response to OMB questions.

The National Nursing Home Survey
(NNHS) will be conducted by personal interviews among a sample of
nursing homes. The survey will provide estimates and
characteristics of nursing homes, residents, and staff. The 2004
NNHS will include a supplement survey of nursing assistants working
in nursing homes.
On behalf of this Federal agency, I certify that

1320.8(b)(3).
