provide change worksheets to reflect decreases in burden resulting
from increased levels of electronic reporting associated with
expansion of the NEDSS platform. OMB extends approval not to
display the expiration date for OMB approval for attachments 8 and
9 for the first year of the collection only, after which CDC will
provide a report to OMB indicating that the forms have been
appropriately updated with the OMB expiration date. Additionally,
CDC will provide a report to OMB after the first year of approval
on the conversion of the race/ethnicity format associated with
NEDSS implementation.

CDC is responsible for the collection
and dissemination of nationally notifiable diseases information and
for monitoring and reporting the impact of epidemic influenza on
mortality. This clearance covers data collection of weekly and
annual morbidity reports from the 57 respondents (50 state, 2 city,
and 5 territorial health departments) and mortality reports from
122 US cities and metropolitan areas.
