Abstract: Child support enforcement agencies, courts, attorneys, private collection agencies, and individuals complete orders and notices to withhold income for child support. PRWORA requires the use of standardized forms promulgated by the Secretary of HHS to ensure uniformity in interstate case processing and ease the burden of compliance for employers. This revised form clearly distinguishes when the form is sent by a state agency, private firms, or others.
