Abstract: The Medicare Prescription Drug, Improvement, and Modernization Act of 2003 establishes a new Medicare Part D program for voluntary prescription drug coverage of, and in some cases subsidies for premium, deductible, and co-payment costs for eligible low-income individuals. Form SSA-1020 is used to apply for these subsidies. The proposed collection consists of the Internet and Intranet (field office screens) versions of form SSA-1020.
