Abstract: This information collection will be performed using an online, web-based survey tool. The survey automates an existing HR form for USAID employees and applies it to USAID contractors to provide emergency contact information, as well as collecting some general work-related information. The information collected will be used to help automate emergency notifications to USAID contractors via a new electronic notification system. The information will also be used as a validation tool for the gathered information stored in various USAID computer systems, which will help facilitate IT and facilities cost recovery effort
