Abstract: VA Form 21-0783 is used to gather specific information from claimants about radiation exposure. The form requests the information that is necessary to verify participation in a radiation-risk activity during military service and, when applicable, obtain a radiation dose assessment. Without this collection of information, VA would not fulfill its statutory duty to assist claimants and would be unable to properly authorize benefits.
