Collection is cleared conditionally upon the associated DoD form,
DD Form 370, being revised to accurately reflect the routine uses
contained in the companion systems of records. The revised
description of the routine uses shall be sufficient to adequately
understand what the information being provided may be used for
while allowing for the limitations of DD Form 370's size. DoD has
30 business days from the date of clearance of this information
collection to provide OIRA with the revised DD Form 370.

Title 10 USC, Sections 504, 505, 508,
and 12102 establish minimum standards for enlistment in the Armed
Forces. This information collection is for reference information on
individuals applying for enlistment in the Armed Forces of the
United States who require a waiver._ The form associated with this
information collection, DD Form 370, "Request for Reference", is
used by recruiters to obtain reference information on applicants
who have admitted to committing a civil or criminal offense. The
