Abstract: SSA will collect information from applicant/recipients of Title II benefits to supplement information provided on SSA's application forms (SSA-1, SSA-2, SSA-4, SSA-5, SSA-7, SSA-10 and SSA-16), when the applicant answers yes to prescreening questions in the applications regarding felony, Federal and State warrants, and violations of probation and parole. SSA will use the information to determine eligibility for benefits.
