Individuals requesting an original or replacement Social Security Number (SSN) card must complete form SS-5 (U.S. residents) or SS-5-FS (U.S. citizens living abroad). Applicants for original or replacement cards must provide evidence to establish their eligibility for the cards. The newly enacted intelligence Reform and Terrorism Prevention Act of 2004 is limiting the number of allowable replacement cards; these forms will be changed to reflect the new limits. Respondents are applicants for new or replacement SSN cards.
