This data collection was designed as a
nationally representative network of primary care service delivery
sites serving vulnerable populations. It will serve as a source of
up-to-date data on patients, encounters, and practitioners that can
be utilized for quality improvement efforts. This system will
collect three levels of data: User Level Data (demographic data),
Practitioner Level Data (education level, clinical specialty), and
Encounter Level Data (site, service, procedure, drug, and diagnosis
codes).
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
