This data collection was designed as a nationally representative network of primary care service delivery sites serving vulnerable populations. It will serve as a source of up-to-date data on patients, encounters, and practitioners that can be utilized for quality improvement efforts. This system will collect three levels of data: User Level Data (demographic data), Practitioner Level Data (education level, clinical specialty), and Encounter Level Data (site, service, procedure, drug, and diagnosis codes).
