Abstract: SBA Form 355 expires July 31, 2006. The Agency is making changes to this form to coincide with its current uses and purposes. The most notable change is in the name of the document from "Application for Small Business Size Determination" to "Information for Small Business Size Determination." The change is intended to inform the public that SBA does not offer requests for small business size determinations, but uses the form for size process and agency requests for size determinations, only.
