Abstract: FCC Form 601 is a multi-purpose form used to apply for an authorization to operate radio stations, amend pending application, modify existing licenses and perform a variety of other miscellaneous tasks in the Public Mobile Services, Personal Communications Services, General Wireless Communications Services, Private Land Mobile Radio Services, Broadcast Auxillary Services, Fixed Microwave Services, Maritime Services (excluding ships) and Aviation Services (excluding aircraft).

Emergency Justfication: The Commisson requests emergency OMB approval to implement a revision to FCC Form 601, Schedule B, in order to obtain information required to be filed by designated entity licensees pursuant to the recently-adopted Commission order. The Commission will review such filing to ensure that a designated entity applicant is in fact qualified to receive the applicable financial benefits and has not entered into inappropriate agreements and/or arrangements with entities that are not eligible for such benefits. This information must be collected at the conclusion of the currently on-going Auction 66 involving wireless spectrum.

Short Statement: The FCC has revised this information collection which added 400 additional respondents. This information is required to be filed by designated entitiy licensees.
