Abstract: This form is submitted to the Food and Nutrition Service's Child Nutrition Payment Center where it is entered into a computerized payment system. The system computes earnings to date and the number of meals served to date and generates payments for the amount of earning in excess of prior advance and claim payments.

Short Statement: The number of respondents have been reduced from 212 to 123 because the Michigan Department of Education assumed full responsibility for the administration of the SFSP in the State of MI. SFSP in MI was formerly administer under ROAP. The reduction from the 610 hours currently approved to the requested 423 hours, resulted in an adjustment of -187 hours.
