Consistent with previous terms of clearance, the data collected using this customer satisfaction survey shall not be used to make generalizations about the customer population. This ICR is approved on the understanding that a web-based version of the survey will be posted on the agency's website within 10 days of OMB approval, as stated in the supporting statement. When next re-printing the paper-based form, the agency will change the burden estimate in the PRA blurb to be consistent with what has been written in the supporting statement.

The current survey has been revised since the last data collections activities. Additionally, the most recent request mode data indicates that the overwhelming number of requests for information come via e-mail or the web. Thus, adjustments have been made to reflect that the survey will be disseminated via e-mail and the web, with mail and telephone interviews representing less than 1 percent of the anticipated responses.
