NTIA is proposing to implement a program to provide $40 coupons to consumers for use towards the purchase of digital-to-analog converter boxes. The converter boxes are necessary for consumers who wish to receive broadcast programming over-the-air using analog-only televisions after the date that full-power stations are required to cease analog broadcasting. (ii) Use of information;
(iii) Burden estimate;
(iv) Nature of response (voluntary, required for a benefit, or mandatory);
(v) Nature and extent of confidentiality; and

If you are unable to certify compliance with any of these provisions, identify the item by leaving the box unchecked and explain the reason in the Supporting Statement.
