Below are the changes to be made to the questionnaire under OMB control number 2080-0068 for 2007. Per OMB’s request, if any minor modifications are to be made each year to the questionnaire, EPA will submit the modified questionnaire to OMB. Below is a summary of the changes for this year’s questionnaire followed by the entire questionnaire.
