Burden Statement - This collection of information is voluntary and will be used to request s submissions for the Transportation Planning Excellence Awards. Public reporting burden is estimated to average 1.5 hours per response, including the time for reviewing instructions searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information. The purpose of the Transportation Planning Excellence Awards Program is to recognize outstanding transportation planning initiatives across the country. Please note that an agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a currently valid OMB control number. The OMB control number for this collection is 2125-XXXX with an expiration date of XXXX.

NOMINATION FORM

Anyone can submit a nomination. However, the nomination must be for a project, process, group or individual involved in a project or process that has made an outstanding contribution to the field of transportation planning and either used U.S. Department of Transportation funding or required U.S. Department of Transportation.


Completed entries must be received on or before February 29, 2008.

Additional Submission Information

  • Entries must include at least two separate professional quality color prints showing the associated project or person(s) nominated. You can provide your photograph as a print or an electronic file. In both cases, the bigger it is the better. Send the largest file you have. If you send it electronically, format the file as a tif or jpeg. The ideal size specifications are: 300 dpi (dots-per-inch or pixels-per-inch) and/or at least 4-inches by 6-inches. If your image is not this big, send it anyway, but understand that it may not be usable. These electronic specifications will make a very large file, and you must put it on a CD. Our email system can not accept a file larger than 3 MG.

  • Photos should be submitted via express mail to Jody McCullough; FHWA Office of Planning; 400 7th St SW, Room 3301; Washington, DC 20590. These photos will be used in publications and marketing material, but will not be forwarded to the panel of judges.

  • If necessary, additional illustrations and other supplemental materials deemed essential for the judges to fully understand the entry can be submitted. Illustrations must include the entry name and a description of the illustration. Submit three (3) sets of all supplemental materials via express mail to: Jody McCullough; FHWA Office of Planning; 400 7th St SW, Room 3301; Washington, DC 20590.

  • Entries may be submitted for multiple categories by completing a separate nomination package (including any supplemental materials) for each category. Undifferentiated submissions to multiple categories will be disqualified.

  • All materials (including photographs and pictures) submitted become the property of the U.S. Department of Transportation and may be used by the Department for its purposes. No entry materials will be returned.
