REQ –

We have no record of railroad compensation for you under the name and social security
account number shown on your recent application for benefits. Consequently, we cannot
determine whether you are qualified for benefits.
If you think our records are wrong and you believe you are qualified, complete and return the
enclosed form. Be sure your name and social security account number are entered correctly.
To be qualified for benefits under the Railroad Unemployment Insurance Act for the general
benefit year July 1, 20XX through June 30, 20XX, you must have had railroad earnings (not
counting over $XXX for any one month) of at least $XXX.00 in 20XX. If 20XX was your first
year in railroad work, you must also have had railroad service in at least five months in 20XX.
