Dear Current Resident,

Your household has been selected for this year’s California Health Survey. This telephone survey is an important opportunity to have your voice heard on health care issues. Your household is part of a scientific sample representing many other households like yours.

We do this survey every two years. This research It helps California learn about the health of its people and the problems they have getting health care. The results may be used to help the people and families in your community.

We recently contactedcalled your home, but it was not a good time for anyone to speak with us. We know your time is valuable. However, we encourage you to take just a few minutes to speak with ustalk to our interviewer when we call again.

Westat, a social science research firm, is conducting this study. In the next week or two, an interviewer from Westat will visit your household to try to reach you. If we happen to stop by at an inconvenient time, please suggest a time that is better for you. If you would like to set an appointment before we stop bycall, you can callontact Westat at their toll-free number, 1-1-888-941-2950.

If the person who is selected from your household agrees to participate in the interview, we will pay $25 for the selected person’s time.
