INTRODUCTION- For the 2009 National Hospital Ambulatory Medical Care Survey (NHAMCS), we are working to expand the sample to include ambulatory surgery centers (ASC). In order to evaluate whether all forms were correctly modified to include ASCs, a pilot test was developed to utilize the forms in pre-production. This expansion is the largest change in the NHAMCS since the survey began in 1992; thus testing these materials is vital to the continued success of the survey.


This evaluation asks some specific questions about the forms, as well as some general questions about your experiences with it. Because the patient record forms may have been completed by the field representative, we realize your involvement may have been limited. However, in our effort to make these forms as useful as possible, we would like your feedback on them.


Please include any comments you feel would help in improving the forms. This is not a test, and there are no right or wrong answers. If any items on the forms are unclear to you, they are probably unclear to others as well. By telling us about any problems, we can work to improve the format for you and other participants. Therefore, the more you can tell us about your experiences with the forms, the better.
