Abstract: In order to receive financial assistance or to amend an existing grant, recipients must complete the Project Agreement Form. The form sets forth the obligations of the State as a consequence of accepting the assistance as well as any special terms and conditions. Amendments to the terms of the grant are accomplishied by completing the Amendment to Project Agreement Form.
