Revised forms have been necessitated by the 2006 reauthorization of the Older Americans Act. The basic forms are electronic and form the basis for the program’s Internet-based data collection system – the SCSEP Performance and Results QPR (SPARQ) system. In addition, ETA has agreed to grantees’ requests for enhanced case management functionality in SPARQ. This collection also contains forms for surveying the customer satisfaction levels of participants, non-profit host agencies, and private or non-profit employers who hire program participants. Finally, moving the collection into ROCIS requires some substantial reorganization and the creation of multible ICs within the ICR.
