Revisions to the Collection Instrument
SSA is requesting formatting changes to the form that will allow faster processing of the SSA-1588 which will lead to better customer service.
The requested changes are as follows:
Move the change of address box from the envelop to the form
Modifications to Question 1:
For Question 1.d. add a segmented box.
Update the Paperwork Reduction Act Statement
